If you’ve been told your IBS is “stress-related” and felt patronised, this page is for you. The research on hypnotherapy for IBS is unusual — both for how strong it is, and for how rarely it’s offered as a first-line treatment.
Gut-directed hypnotherapy is recommended by NICE, the National Institute for Health and Care Excellence, as a treatment for IBS that hasn’t responded to dietary changes or medication. It’s one of the most evidence-based applications of hypnotherapy in existence — and it was developed originally in Manchester, a few hours up the M6 from my Birmingham clinic.
The gut and the brain communicate constantly via the vagus nerve, in what’s called the gut-brain axis. In IBS, this signalling appears to be amplified — normal digestive sensations get registered as pain, mild gut activity feels like cramping, gas that wouldn’t bother most people becomes severe bloating.
Gut-directed hypnotherapy recalibrates this signalling. Brain imaging studies have shown reduced activation in the parts of the brain that process gut pain after a course of hypnotherapy — and the calmer signalling persists long after sessions end.
It’s not “all in your head” in the dismissive sense. The pain is real. The volume control on the pain is in the nervous system, and hypnotherapy reaches that volume control.
Gut-directed hypnotherapy was developed by Professor Peter Whorwell at the University of Manchester. The NICE-aligned protocol involves:
Response rates in research have been around 70–80% — meaning a significant reduction in symptoms in three quarters of patients. Follow-up studies have shown improvement persisting for years after treatment ends, which is unusual for IBS treatments.
The approach works across all types — IBS-D, diarrhoea-predominant; IBS-C, constipation-predominant; and IBS-M, mixed. It works for IBS related to specific food triggers and IBS that flares with stress.
Please make sure your IBS has been properly investigated before starting hypnotherapy. Many Birmingham clients come to me after diagnosis from a gastroenterologist at QE Birmingham, BMI Edgbaston, or Spire Parkway, often after FODMAP diets, antispasmodics, or other approaches haven’t fully resolved symptoms.
If you’re under a gastroenterologist, keep seeing them. Hypnotherapy works alongside medical care, not in place of it.
